The 13 questions

Open a blank document, put each question at the top of its own block, and just answer. Nobody grades this. It only has to be true.

If a program director could only remember three words about you after reading your whole application, which three words do you want them to be?
Look at your CV and pick the four to six lines you're proudest of. For each one, tell me what it actually took to get it, the part that doesn't show on paper.
Walk me through how your main interest (research area, cause, skill, whatever you named in question 1) actually developed. Where did it start, and what were the two or three steps that deepened it?
Now forget medicine for a minute. Tell me about a place, a routine, or a ritual from your life before med school that shaped how you work. A garage, a kitchen table, a practice field, a family shop. What did it look like, sound like, smell like? Who was there?
Who taught you the most before medicine ever entered the picture? What did they actually do, day to day, that you find yourself copying now?
Out of that place and that person, what two or three things did you learn that you still use every week in the hospital? Say each one in a single sentence, like you're telling a friend the moral of a story.
What was the moment urology stopped being one option among many and became the plan? Not the polished version, the real one.
What is it about the way urology works, the patients, the problems, the culture of the field, that fits the way your brain works? Where does your question 1 answer show up inside urology?
Which urology patients do you find yourself still thinking about after clinic, and why those?
What do you do when you're nowhere near a hospital, and what has it quietly taught you that shows up in your clinical work?
Has anything made this road harder for you than it looks from the outside? One or two sentences, and then tell me what it built in you. (Optional. Skipping this one is a valid answer.)
Ten years from now you're an attending. What are you known for, and what did you add to the field that wasn't there before?
Last one. Read your answer to question 1 again. Does everything you wrote above point at those three words? If something doesn't fit, which answer would you change, the three words or the story?
